This Privacy Policy explains what information we collect when you use the Website, how we use and protect it, and the choices and rights you have — including your rights under California law.

2. INFORMATION WE COLLECT
Information you give us. When you submit an inquiry, request information, or contact us, we collect what you provide — typically your name, email address, phone number, preferred location, how you heard about Era, and your referring physician, if you have one. If you choose to share anything about your health in a message to us, we treat that information with heightened care, as described in Section 7.
Information collected automatically. Like most websites, we automatically collect certain technical information when you visit: your IP address, browser and device type, operating system, the pages you view, how long you stay, and the website that referred you. Some of this is collected through cookies and similar technologies, described in Section 4.
Information from others. If a physician refers you to Era or a current member introduces you, we may receive your name and contact information from them.
In the terms used by California law, over the past 12 months we have collected: identifiers (such as name, email, phone number, IP address); professional information (such as your referring physician); internet activity (such as browsing behavior on our Website); general geolocation (inferred from IP address); inferences (such as your interest in our services); and, only if you volunteer it, sensitive personal information (such as health information you include in an inquiry).

We do not sell your personal information, and we do not share it with third parties for cross-context behavioral advertising. We have not done either in the preceding 12 months. Because we don't sell or share personal information, and because we have no actual knowledge of selling or sharing the personal information of anyone under 16, we do not offer an opt-out — there is nothing to opt out of.

6. YOUR CALIFORNIA PRIVACY RIGHTS
If you are a California resident, the California Consumer Privacy Act (as amended by the California Privacy Rights Act) gives you the following rights:

If that ever changes, we will give you the right to limit its use.

We will confirm receipt within 10 business days and respond within 45 days (extendable to 90 with notice). To protect your information, we will need to verify your identity — usually by matching information you provide against what we already have. You may also designate an authorized agent to make a request on your behalf; we will ask for written proof of their authority and verify your identity directly.
Shine the Light. California Civil Code Section 1798.83 lets California residents ask companies annually for a list of the third parties to whom they've disclosed personal information for those parties' direct marketing purposes. We don't disclose personal information for third parties' direct marketing, so no such list exists but you're welcome to ask at the contact information above.

7. HEALTH INFORMATION YOU SHARE WITH US
Sometimes people tell us about their health before becoming a member in an inquiry form, an email, or a call. When you do, we treat that information as sensitive: we use it only to respond to you and to connect you with our clinical team, we share it only with the people who need it to do that, and we never use it for marketing.
Once you become a member and receive care, your health information is protected health information under HIPAA and California's Confidentiality of Medical Information Act, and our Notice of Privacy Practices governs from there.

8. TEXT MESSAGING
If you give us your mobile number and agree to receive text messages, we may text you about your inquiry or scheduling. Message frequency varies; message and data rates may apply. Reply STOP at any time to opt out, or HELP for assistance.
Your mobile number and your consent to receive texts will never be shared with or sold to third parties for their marketing purposes.
